Getting Started
If you would like to learn more about our practice or discuss your individual needs, we offer free 15-minute phone consultations to see if we would be a good fit. If you would like to become a client of ours, please call or email our office and inquire if we are accepting any new clients.
Referrals
If you are a provider or a case manager in a higher level of care and would like to refer a patient for nutrition counseling, please either call or send a referral to our office.
Session Fees
Our private pay rates are $180 for the initial session and $120 for follow-up sessions.
Insurances Accepted
RestorED Nutrition Therapy is contracted/in-network with the following insurance payers: Aetna, Blue Cross Blue Shield PPO, Cigna, Healthcare Highways, Health Choice, Humana, and Tricare East. If you would like to use your insurance to help pay for sessions, please call your insurance company prior to our first session in order to verify nutrition benefits of your plan. Please ask us about our form “Checking your Insurance Benefits” that outlines this process and what to ask.
Required Paperwork
New client forms and other required documents will be sent to you for completion in the Client Portal. Once completed, we will schedule your first session!
